Calculation Methodology

  1. 1
    From the CMS Provider Information dataset, read monthly provider information files and extract the state column (State) and the Urban column for each facility record. The Urban column is available from October 2025 onwards, where Urban=Y indicates an urban location and Urban=N indicates a rural location. Exclude records where state is AS, GU, MP, PR, or VI.

    Aggregate the number of nursing home facilities by state and urban/rural classification for each month where the Urban column is present.
  3. 3
    Calculate the yearly mean of monthly counts per state and location type to produce annualized counts.
