Overview
Monthly consolidated archive of all CMS Provider Data Catalog (PDC) tables for U.S. nursing homes and rehab services. Provider Information includes 5-star ratings (overall, health inspection, quality measures, staffing), certified bed counts, average daily resident census, ownership type, certification, chain affiliation, urban/rural classification, staffing hours per resident per day (reported, case-mix adjusted, and weekend), staff turnover rates, and penalty history. Additional tables cover state and national averages, health and fire safety inspection deficiencies with scope/severity, MDS-based and Medicare claims-based quality measures (long-stay and short-stay), detailed ownership records, inspection dates and survey summaries, and citation code lookups. The archive also includes Skilled Nursing Facility Quality Reporting Program (SNF QRP) data with IMPACT Act quality measures at national and provider levels, and SNF Value-Based Purchasing (VBP) facility performance scores and incentive payment multipliers.
Data Limitations
Data reflects CMS administrative records and provider self-reported information at the time of each monthly snapshot. Facilities that do not participate in Medicare or Medicaid are excluded. Quality ratings are derived from health inspections, staffing data, and quality measures that may lag actual conditions. Urban/rural classification is only available from 2025 onwards.
